standing orders	empowers nurse to initiate actions that ordinarily require the order or supervision of a physician (standard orders for a narcotic overdose that specify the agents the nurse is to administer to reverse respiratory depression in an emergency
the legal truth	it wasn't done if it wasn't documented.
When a patient does not follow the plan of care despite your best efforts	1. identify why the patient is not following the therapy. is the plan of care right for the patient? 2. if plan of care is adequate, identify/remedy the factors contributing to the patient's noncompliance
