What is polymyalgia rheumatica? Polymyalgia rheumatica is an inflammatory condition that causes pain and stiffness in the shoulders, pelvic girdle and neck.
What are the core symptoms of PMR? Bilateral shoulder and/or pelvic girdle pain; may radiate to elbow/knees. Stiffness lasting >=45 mins in the morning/after periods of rest Pain which is worse with movement and interferes with sleep
What are additional features of PMR? - Systemic symptoms - low grade fever, anorexia, weight loss and depression - Bilateral upper arm tenderness - Peripheral MSK signs e.g. carpal tunnel, asymmetrical peripheral arthritis (predominately affecting the knees and wrists) - Swelling with pitting oedema of hands, wrists, feet and ankles
What is the initial management of PMR? Oral prednisolone 15mg daily - review response after 1 week and dose after 3-4 weeks.
What condition is often associated with PMR? Giant cell/temporal arteritis
